Too many colon tests use costly sedation: Study

CHICAGO (AP) ? Few people want to be wide awake during their colonoscopy exams, but new research suggests too many are getting extra sedation treatment, costing as much as $1 billion yearly in potentially needless services.

Use of anesthesiologists to monitor sedation during colonoscopies and other digestive imaging tests has more than doubled in recent years, and they're used most often for low-risk patients who typically don't need the extra help, the study authors said.

"These services are not harming patients. They're basically giving them a luxury that is not strictly necessary," said the study's lead author, Dr. Soeren Mattke, a senior Rand Corp. scientist. That matters at a time when policymakers are trying to rein in rising medical costs, the authors said.

Patients usually are briefly sedated for a colonoscopy, and some kinds of sedation require monitoring by specialists. That includes use of propofol, a powerful intravenous drug that can cause deep sedation and was implicated in pop star Michael Jackson's death. Anesthesiologist-monitored sedation, with or without propofol, is recommended for high-risk patients, including those who are old or sick or previously had complications with anesthesia.

While propofol sedation is also sometimes given to low-risk colonoscopy patients, the study authors suggest that's often unnecessary. Drugs usually recommended for these patients typically cause lighter sedation, though most patients don't remember anything about the exams afterward. These drugs can be given by the doctor doing the exam, but the study suggests they're often also being given and monitored by anesthesiologists.

The researchers analyzed insurance claims data on more than 6 million U.S. adults who had the colon exams or imaging scopes of the upper digestive tract between 2003 and 2009. The tests are done to screen for colon cancer, acid reflux and other illnesses.

When the study began, 14 percent of these tests included an anesthesiologist. That climbed to more than 30 percent by 2009. The portion of this extra sedation treatment given to low-risk commercially insured patients remained constant during the study and fell slightly in Medicare fee-for-service patients. But the study authors said far too many low-risk patients are still getting this treatment ? more than three-fourths of the commercially insured patients and two-thirds of the Medicare group.

The extra treatment added an average of about $500 to an insured patient's bill in 2009, and $150 to a Medicare bill. In 2009, about 3 million colonoscopies and other digestive scoping tests were done in low-risk patients but included anesthesia services, amounting to $1 billion in potentially unnecessary costs, the study authors estimated.

The study appears in Wednesday's Journal of the American Medical Association.

While some insurance policies exclude coverage for anesthesiologist monitoring for low-risk patients undergoing these exams, insurers sometimes pay for it, Mattke said.

Because of doctor backlash, Aetna, one of the nation's largest health insurers, has indefinitely delayed a policy it tried to implement in 2008 that would have excluded this coverage for low-risk patients, said Dr. James Cross, Aetna's chief of national medical policy and operations.

Reasons for the frequent use of anesthesiologists during these exams vary. Sometimes low-risk patients undergoing colonoscopies and other digestive scoping exams request propofol because they want to be totally unconscious and have heard that it wears off quickly and doesn't cause grogginess, unlike other sedatives, said Dr. John Vargo, a spokesman for the American Society for Gastrointestinal Endoscopy and a digestive specialist at the Cleveland Clinic.

Propofol requires careful monitoring because it has "a narrow window between providing deep sedation and making people stop breathing," and unlike other sedatives, there's no rescue drug to reverse its effects, said Dr. Norm Cohen, vice president for professional affairs at the American Society of Anesthesiologists.

The rising use of anesthesiologists may partly reflect more obese patients and users of Vicodin and other opiate-based prescription drugs, Cohen said. Both may be missed under coding used in the study, but they should be considered at risk because sleep apnea that often accompanies obesity makes sedation trickier, and users of opiate painkillers often require higher than usual doses of sedation, he said.

Vargo said doctors who do the exams can be trained to use propofol in healthy patients, but a journal editorial said some prefer anesthesiologist assistance because it allows them to focus on the colon exam, and if something goes wrong with the sedation, they may not be held legally accountable.

Dawn Meehan, 42, an Orlando, Fla.-area teacher's assistant and writer, had a colonoscopy last month under deep sedation monitored by an anesthesia specialist; her insurance covered everything. Colonoscopies to screen for colon cancer usually aren't recommended?until age 50 but Meehan had the exam because of symptoms for a common?digestive disease.

She was a low-risk patient and said if her colonoscopy doctor had offered it, she might have chosen light sedation. But even though the extra sedation is more costly, Meehan said patients who want it should get it, because otherwise some might "shy away from getting screened."

Holding a gun makes you think others are too, new research shows

ScienceDaily (Mar. 21, 2012) ? Wielding a gun increases a person's bias to see guns in the hands of others, new research from the University of Notre Dame shows.

Notre Dame Associate Professor of Psychology James Brockmole, who specializes in human cognition and how the visual world guides behavior, together with a colleague from Purdue University, conducted the study, which will appear in an upcoming issue of the Journal of Experimental Psychology: Human Perception and Performance.

In five experiments, subjects were shown multiple images of people on a computer screen and determined whether the person was holding a gun or a neutral object such as a soda can or cell phone. Subjects did this while holding either a toy gun or a neutral object such as a foam ball.

The researchers varied the situation in each experiment -- such as having the people in the images sometimes wear ski masks, changing the race of the person in the image or changing the reaction subjects were to have when they perceived the person in the image to hold a gun. Regardless of the situation the observers found themselves in, the study showed that responding with a gun biased observers to report "gun present" more than did responding with a ball. Thus, by virtue of affording the subject the opportunity to use a gun, he or she was more likely to classify objects in a scene as a gun and, as a result, to engage in threat-induced behavior, such as raising a firearm to shoot.

"Beliefs, expectations and emotions can all influence an observer's ability to detect and to categorize objects as guns," Brockmole says. "Now we know that a person's ability to act in certain ways can bias their recognition of objects as well, and in dramatic ways. It seems that people have a hard time separating their thoughts about what they perceive and their thoughts about how they can or should act."

The researchers showed that the ability to act is a key factor in the effects by showing that simply letting observers see a nearby gun did not influence their behavior; holding and using the gun was important.

"One reason we supposed that wielding a firearm might influence object categorization stems from previous research in this area, which argues that people perceive the spatial properties of their surrounding environment in terms of their ability to perform an intended action," Brockmole says.

For example, other research has shown that people with broader shoulders tend to perceive doorways to be narrower, and softball players with higher batting averages perceive the ball to be bigger. The blending of perception and action representations could explain, in part, why people holding a gun would tend to assume others are, too.

"In addition to the theoretical implications for event perception and object identification, these findings have practical implications for law enforcement and public safety," Brockmole says.

Interview With Switched at Birth?s Sean Berdy

There are many reasons to watch the ABC Family show Switched at Birth. It is an amazing show that brings the deaf and hearing communities together, plus it has cast full of super talented people. One of my favorite actors on the show is Sean Berdy, who plays Emmett Bledsoe and the other day I was lucky enough to get to interview him. Hunky Sean Berdy is making a name for himself on Switched at Birth. His character Emmett has a James Dean quality about him that is making fans all over fall in love with both Sean and his alter ego. Recently I was fortunate enough to participate in a Q&A interview session with Berdy, along with his interpreter. I tell you he is just as awesome in real life as he is on screen. Sean was so open during the interview talking about everything from being deaf, to his acting, to his love of the drums to finally what the show means it him. He revealed that he is honored to be part of a show that so uniquely shows not only the hearing and deaf community intertwined but also shows authentic deaf characters and their perspectives. Diane Ravitch: In Defense of Facing Reality

I recently wrote two review articles for the New York Review of Books about the teaching profession. The first was a review of Pasi Sahlberg's Finnish Lessons, about the exceptional school system of Finland, which owes much to the high professionalism of its teachers.

The second of the two articles was a review of Wendy Kopp's A Chance to Make History, and it focused on her organization, Teach for America.

I expressed my admiration for the young people who agree to teach for two years, with only five weeks of training. But I worried that TFA was now seen -- and promoting itself -- as the answer to the serious problems of American education. Even by naming her book A Chance to Make History, Wendy Kopp reinforced the idea that TFA was the very mechanism that American society could rely upon to lift up the children of poverty and close the achievement gaps between different racial and ethnic groups.

Wendy Kopp responded to my review of her book with a blog called "In Defense of Optimism." She wrote that:

... over the last twenty years we in the United States have discovered that we don't have to wait to fix poverty to dramatically improve educational outcomes for underprivileged students. In fact, there's strong evidence that one of the most effective ways to break the cycle of poverty is to expand the mission of public schools in low-income communities and put enormous energy into providing children with the extra time and support they need to reach their potential.

Now I certainly agree with Kopp that schools are enormously important, and that it's vital to have talented educators working in them. We both want to see the day when every child has access to an excellent education. She believes that the teachers and the leaders produced by TFA have figured this out. I disagree. I think that the lesson of Finland and other high-performing nations is that we must improve the teaching profession, so that career educators receive the respect and working conditions they need to succeed, and we must also reduce poverty.

If it were true that we now know how to break the cycle of poverty, poverty would be declining. But poverty is growing in the United States; child poverty is more than 20 percent and rising. Among the world's advanced nations, we are number one in child poverty. It's facile to blame schools and teachers, but more realistic to recognize that poverty is a reflection of economic conditions. Schools cannot create jobs, provide homes for the homeless, or change the economy.

Kopp is right that TFA has become a training ground for leaders. Some of its alumni have moved into high-level positions, like Michelle Rhee, former chancellor of the public schools of the District of Columbia, who now works closely with the nation's most conservative governors to strip teachers of due process rights and to promote charter schools, vouchers, and for-profit education corporations. Another TFA alum, John White, Commissioner of Education in Louisiana, advances the same hard-right agenda for Governor Bobby Jindal.

In my reviews, I contrasted the five-year preparation of teachers in Finland with the American hodge-podge approach to the recruitment and training of teachers. In the U.S., states offer many ways to become a teacher, and our non-system has produced low standards for entry and a revolving door, with 40-50 percent leaving in their first five years of teaching. Finnish teachers are highly respected and seldom leave their profession.

Kopp dismisses Finland as a model because less than 4 percent of its children are poor. But that's part of the story of their success and should not be waved aside as unimportant. Teacher professionalism is also part of Finnish success. In this country, our public school teachers are constantly criticized and disrespected, and few are recognized for their dedication and hard work despite budget cuts, growing class sizes, and a hostile media. So long as the attacks on teachers continue, so long as the politicians continue defunding the schools, and so long as our society continues to tolerate high levels of child poverty and intense racial segregation, we will continue to have low-performing students and "failing" schools.

We will have to learn to hold two ideas in our heads at the same time: We must both reduce poverty and improve our schools. We cannot fix our schools without strengthening the teaching profession and addressing the social conditions that shape their outcomes.

Clooney comes under rocket fire in the Sudan

Some actors went back to their mansions and Beverly Hills lifestyles post-Oscars; George Clooney went to the Sudan. And as he told TODAY's Ann Curry, who interviewed him just hours after his return, it wasn't for a handshake and a photo opportunity. Clooney, and his camera crew, were under fire.

"We showed up in one village and there was -- 150 people had come out and were cheering for us to come," he said in the interview that aired Wednesday morning. "Clearly somebody had told them that we were coming. ... And then all of a sudden everyone just started running."

Video clips from the incident showed Clooney talking about heading into the caves with villagers for safety as rockets sailed overhead. He told Curry the rockets were "close enough to feel it."

He said, in the past month, 39 people of the 1,000 in a village he visited had been killed, and he pointed the finger at Sudan's government, including President Omar Hassan Ahmad Al-Bashir. "The same three guys ... who were charged for war crimes in Darfur are the exact same people bombing innocent people."

But only non-Arabs, he pointed out. "This is ethnic cleansing," he said. "That simple."

Clooney has long been an activist working to bring light to the situation in Darfur, and now North and South Sudan. This was his sixth trip to the region. He noted that Americans needed to be not just aware of suffering over there, but of the U.S.'s role. Ethnic cleansing aside, he said, it's ultimately about oil: South Sudan has the oil, North Sudan has the refineries.

"Right now what is going on in the Sudan changes the cost of your gas every single day of your life," he said. "If for no other reason than your economic interests, there is plenty of reason to make sure that your government is involved in trying to secure some form of peace."

But in addition to economics, Clooney said he expects Americans to empathize with simple human suffering and the will to survive.

"You feel an enormous, enormous sense of responsibility," he said. "We're only successful as a human race by how we look out for the people who can't look out for themselves."

Clooney will be in Washington, D.C. today to testify before the Senate Foreign Relations Committee about what he saw in Sudan; afterward, he will meet with Secretary Hillary Clinton and President Obama in the Oval Office.

Norwood, Mass.?The addition of 54 new one-bedroom apartment homes at Norwest Woods in Norwood, Mass., is just the latest in an ongoing success story authored by Chestnut Hill Realty, one of New England?s leading residential real estate firms. The new apartment homes represented almost 10 percent of all new apartments built last year in the Greater Boston area, Chestnut Hill Realty reported.

Chestnut Hill Realty (CHR) acquired the 21-acre, 322-unit Norwest Woods in 1999, and has substantially revitalized the community in the years since. But there remained a problem. Approximately 80 percent of the residences were two- and three-bedroom units, the remaining 20 percent one-bedrooms.

?Taking land that had no land cost and building new one-bedrooms let us appeal to a much larger audience,? Jerry Rappaport, CHR managing director, tells MHN. ?We could attract a younger, higher-income demographic with new construction.?

CHR priced the new one-bedroom residences at higher prices than the 30-year-old two-bedrooms, he adds. But the company has been able to command more in rent for the two-bedrooms once renters who have experienced the one-bedrooms opt to move up to Norwest Woods residences with more space.

Among the new one-bedroom residences are a number of luxury loft units on the third floor. ?Young professionals like them because they get storage with the loft, or the opportunity to create a home office or guest bedroom,? Rappaport says.

The new one-bedrooms are coming online just as the population of Boston metropolitan area renters is increasing substantially.
